Space & Special Event Request Form

Request form must be submitted a minimum of four (4) weeks prior to the event.
*The 3rd and 4th floors have two basketball courts, also called "gyms." If your event requires "two gyms," request the 3rd or 4th floor. Space is contingent upon availability, and is not guaranteed.
Please note if yes you will incur a House Keeping Fee.
Event Staff: All special events are required to have at least two (2) event staff. Some events, based on the nature and size of the event, will require more. Event staff will be responsible for handling logistical needs during the event. University Police (UPD): All events require the presence of UPD and/or EMeRG to ensure safety and crowd control. LHWC staff will schedule the necessary number of UPD officers based on the space request. Management & Space: Management and space fees are charged, based on the amount of space used, for all events held in the Lerner Health and Wellness Center.
• We accept Visa/MasterCard, GWorld card, cash/check and GW departmental journal transfer for payment. • A university departmental journal transfer is the “preferred” method of payment. • Checks should be made payable to ‘The George Washington University’. • Payments should be coordinated with the Assistant Director of Scheduling & Special Events, and brought to the Campus Recreation Office on the 2nd floor of the Lerner Health and Wellness Center.
